Layanan Reset dan Perubahan Akun DAPODIK

Assalamu’alaikum Warahmatullahi Wabarakatuh

Bagi lembaga yang melakukan pergantian operator, disarankan juga melakukan pergantian user dan password dapodik maupun Verval. Berikut adalah persyaratan untuk pengusulan user dapodik :

  1. Surat permohonan user dapodik ditujukan kepada Kepala Dinas Pendidikan dan Kebudayaan Kabupaten Demak yang didalamnya disertakan alasan perubahan user dan password tersebut. Surat permohonan dalam bentuk hardcopy dan softcopy;
  2. Format usulan User dan Password Dapodik, format bisa diunduh di disini;
  3. Surat Penugasan sebagai operator, surat penugasan tersebut wajib mencantumkan Nama, Tempat Tanggal Lahir, Alamat, Pendidikan Terakhir, Email : Surat Penugasan dalam bentuk hardcopy dan softcopy ;
  4. Softcopy wajib berasal dari berkas Asli, tidak boleh fotocopy. Disimpan dalam format pdf. Kecuali untuk Format Usulan User Dapodik wajib dalam bentuk pdf dan jpg atau gambar selanjutnya dikirim ke ult.lpmpjateng.go.id  sedang hadrcopy dikirim ke Admin Dinas Pendidikan dan Kebudayaan Kabupaten Demak Rangkap 2 (1 Asli, 1 fotocopy legalisir KS).

Jika ada kesulitan bisa ditanyakan melalui fasilitas WA Admin Dinas/Lampu Sentir yang telah tersedia di Laman ini pada jam kerja. Demikian atas perhatian saudara disampaikan terima kasih.

ALUR PELAYANAN

  1. Surat Permohonan Akun/Reset atau Update User DAPODIK;
  2. Formulir Pengajuan Perubahan User dan Password Dapodik;
  3. Surat Penugasan sebagai operator sekolah;
  4. Softcopy di upload di Lampu Sentir (Layanan Masyarakat Terpadu Secara Terintegrasi);

  • Proses dalam Penyelesaian Permohonan Pengajuan Akun Baru, Reset atau Update Akun DAPODIK  dilakukan setelah pemohon mengajukan berkas dan memenuhi persyaratan yang telah ditentukan;
  • Waktu Penyelesaian dilaksanakan paling lambat 3 (tiga) hari kerja sejak diterimanya permintaan, petugas permohonan Pengajuan Akun Baru, Reset atau update Akun DAPODIK baru menyampaikan dokumen dan petugas dapat memperpanjang waktu paling lambat 1 (satu) bulan kerja;
  • Layanan atas permohonan Reset atau perubahan Akun Dapodik Baru dilakukan secara langsung;
3 Hari kerja Waktu Penyelesaian dilaksanakan paling lambat 1 (satu) bulan kerja sejak diterimanya permintaan;
Tidak dipungut biaya (GRATIS);
  • Akun DAPODIK Aktif dan dapat di gunakan untuk Akses Layanan DAPODIK;
